2012 NATIONAL SALES CONFERENCE

HM Insurance Group (HM) has a strong corporate commitment to our strategic producers and partners. As a way to strengthen our relationships, we offer producers the opportunity to qualify for our National Sales Conference.

During the conference, attendees have an opportunity to spend time with HM's senior leadership. Attendees also can enhance their knowledge of HM and the group insurance industry in general, as well as provide valuable input regarding their needs in the marketplace.

The 2012 HM National Sales Conference will be held April 29 – May 3, 2012. Invitees and their guests can qualify to join us at The Four Seasons Hotel in Vancouver, British Columbia, a city renowned for its majestic mountains, beautiful parks and waterfront, history and thriving social and business community.

In addition to the breathtaking accommodations, attendees will enjoy exciting activities during the day and entertaining events in the evening.

2011 PRODUCER CONFERENCE QUALIFICATION REQUIREMENTS

DIAMOND CLUB

Diamond Club members and a guest will enjoy a five-day, four-night stay at the HM 2012 National Sales Conference.

A. NEW BUSINESS QUALIFICATION CAN BE ACHIEVED IN ONE OF TWO WAYS:
Qualifying period: All new production must have effective dates between February 1, 2011, and January 31, 2012

1. Stop Loss - New Business
  • Achieve an invitation by writing $800,000 of new Stop Loss annualized premium with two new cases
  • Additional invitations will be awarded for each additional $1.2 million in new Stop Loss annualized premium and three additional new cases
2. Combination HM and United Concordia Dental (UCD) – New Business
  • Achieve an invitation by writing $500,000 of new annualized HM premium AND $700,000 of new UCD annualized premium.
  • Invitations are limited to one producer per qualifying firm.

B. PERSISTENCY RESULTS CAN QUALIFY FOR AN ADDITIONAL INVITATION:

Stop Loss – Existing Business

Must achieve both of the following:

  • Retain 85% of your HM policies in-force on December 31, 2011, subject to a minimum of $2.0 million and eight in-force cases
  • Write $250,000 of new HM annualized premium

PLATINUM CLUB

Platinum Club members will enjoy a three-day, two-night bonus extended stay and upgrades at the 2012 National Sales Conference. Qualifi cation can be achieved in two ways:

1. Stop Loss - New Business
  • Write $2.75 million in new Stop Loss and/or Worksite annualized premium with six new name lines
2. Stop Loss – Existing Business
  • Retain 90% of your HM policies in-force on December 31, 2010, subject to an in-force premium minimum of $3.5 million as of December 31, 2011

Notes

  • Must be the named Producer of Record to receive full credit.
  • Aggregate Stop Loss premium will be credited toward qualification; the aggregate case count will not be credited.
  • Not all products are available in all states. This program is subject to individual state approval.
  • Only one Platinum Club qualifier per agency.
  • HM Insurance Group will act as final authority for any questions that may arise in the interpretation and/or calculation of any of the provisions of the override program described above. All final calculations will be based on HM Insurance Group records.
  • HM Worksite products include HM Worksite Advantage (Critical Illness, Accident, Disability Income and Term Life) and HM Care Advantage, a Limited Medical Benefit plan.
